The following is a review of all paperwork and record keeping necessary to properly document your clients. We suggest that practitioners keep a small file for each client that you see with the following elements. Students may want to do this for student practice clients as well.

Initial Client Info Form

Release, Waiver and Informed Consent – Keep a copy for your file and provide one for the client.
Client Information Form – This was distributed during the Harmonyum training weekends.

Consent to Treat a Minor Form – For all clients under 18 years of age.

Note Page – It is beneficial to note which level of Harmonyum was done as well as any other pertinent information about the appointment.

Payment Record – This should include each date of service, the amount paid, payment method.

This level of organization will help you to maintain an important level of clarity in your practice.
